Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you need to visit the surgery to complete a travel questionnaire form.  We require each individual traveller to complete a questionnaire.   Upon completing the form, hand it to reception and it will be passed to the Practice Nurse for review.  We advise patients to wait 1 week from handing in the form to contacting us to discuss whether an appointment is required.  If an appointment is required, our reception team will make this form you in accordance with the instructions that our Practice Nurse will have given when reviewing your form.

Travel vaccines have to be ordered as they are not a stock vaccine.  This means that we require patients to complete a travel questionnaire form at least 6 weeks in advance of travel, whenever possible.  If you are travelling with less than 6 weeks notice, please contact us for advice on whether we can accommodate your vaccine request.

Travel vaccines is an additional service that we offer.  Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
